Acknowledgement of Risk and Waiver of Liability & Media Release
Please read this Acknowledgement of Risk and Waiver of Liability & Media Release carefully and in its entirety, as it is required for participation and is a binding legal document. *If you are under the age of 18, this form must be signed by you as the participant AND by your parent or legal guardian.
I acknowledge that participating in the Penns Valley Conservation Association (PVCA) Tunnel Trot Fun Run/Walk is a potentially hazardous activity and I should not participate unless I am medically able and properly trained. I acknowledge and assume all risks associated with this event including but not limited to; traffic on the course route, falls, contact or collisions with other participants, risks related to the condition of the course, obstacles, curbs, cars, uneven pavement, potholes, rocks, and objects on the course surface. Knowing and appreciating these risks I hereby declare for myself, my heirs, representatives, or anyone else claiming on my behalf covenant not to sue, and waive, release, and discharge PVCA employees, volunteers, sponsors, and anyone else acting for or on behalf of the Tunnel Trot Fun Run/Walk from any and all claims of liability for death, personal injury, or damage of any kind arising out of my participation in this run. With my participation in the Tunnel Trot Fun Run/Walk, I consent to emergency treatment in the event of injury or illness should it be necessary.
Media Release
I grant full permission to PVCA and/or any person or entity authorized by it to use my name, age, finish place, and finish time, along with any photographs, recordings, or any other record of this event for marketing/promotional use and in the public domain.
My signature (or electronic equivalent) on the Tunnel Trot Fun Run/Walk Acknowledgement of Risk and Waiver Liability & Media Release confirms that I have read this document in its entirety and understand and agree to all terms and conditions set forth herein.
REQUIRED FOR ALL PARTICIPANTS UNDER 18 YEARS OF AGE
Parent/Guardian Authorization for Medical Treatment and Consent Agreement
As the parent or legal guardian of the above-named minor participant, I acknowledge that my dependent and I have read, understand, and agree to the terms and conditions set forth for this event. I hereby give my consent to the participation of my dependent in the Tunnel Trot Fun Run/Walk, and to receive medical treatment should it be necessary. I further agree to hold harmless, indemnify and defend PVCA from and against all claims, demands or suits that my dependent or I has or may have.
